非特异性免疫YZ 免疫YZZL方法包括抗原非特异性和抗原特异性免疫YZ。类固醇、环孢素 (cyclosporin) 和硫唑嘌呤是目前临床常用的非特异性免疫YZ剂。类固醇的作用机制是: ①YZ巨噬细胞细胞因子 (IL - 1, IL - 6, TNF) 的转录和分泌; ②YZ T 细胞产生的能放大淋巴细胞和巨噬细胞反应的细胞因子; ③YZ IL - 2 产生和其与受体的结合; ④阻断巨噬细胞对淋巴细胞来源的刺激信号的反应; ⑤YZ前列腺素的合成; ⑥影响抗原提呈细胞的功能, 减少 MHC 抗原的表达。环孢素是土壤真菌产生的大环内酯类化合物, 其作用机制是通过干扰细胞因子基因的激活而YZ Th 细胞产生细胞因子, 直接或间接地减少 IL - 2 受体在淋巴细胞上的表达。另一些大环内酯类化合物, 如他克莫司和西罗莫司 (雷帕霉素) 也具有免疫YZ特性。他克莫司YZ Th 细胞产生细胞因子的方式与环孢素相似。西罗莫司干扰 IL - 2 受体细胞内的信号转导通路, 因此防止了 IL - 2 依赖的细胞因子活化。硫唑嘌呤是抗增殖药物, 是 6 - 巯基嘌呤的类似物, 它可以掺入正在分裂的细胞的 DNA, 从而阻止细胞进一步增殖。霉酚酸 (mycophenolate acid, MPA) 衍生物等新的抗增殖药物正在研究或已投入临床应用。虽然这些药物单独应用有效, 但通常要求高剂量, 从而增加了毒副作用。多种免疫YZ剂组合使用, 由于影响了同一免疫途径的不同阶段而具有协同效应, 而且可使每种药物的剂量减少, 相应减少了毒副作用。环孢素的应用获得了非常好的临床效果 (肾、心、肝移植一年接受率为 85%～90% )。
