如多数传染病病人在有临床症状时能排出大量病原体,威胁周围人群,是重要的传染源。但有些病人如百日咳患者,在卡他期排出病原体较多,具有很强的传染性,而在痉咳期排出病原体的数量明显减少,传染性也逐渐减退。又如,乙型肝炎病人在潜伏期末才具有传染性。
一般说来,病人在恢复期不再是传染源,但某些传染病(伤寒、白喉)的恢复期病人仍可在一定时间内排出病原体,继续起传染源的作用。
病原携带者指已无任何临床症状,但能排出病原体的人或动物。携带者有病后携带者和所谓健康携带者两种。前者指临床症状消病毒、机体功能恢复,但继续排出病原体的个体。这种携带状态一般持续时间较短,少数个体携带时间较长,个别的可延续多年,如慢性伤寒带菌者。所谓健康携带者无疾病既往史,但用检验方法可查明其排出物带病原体。这种人携带病原体的时间一般是短暂的。
病动物也是人类传染病的传染源。人被患病动物(如狂犬病、鼠咬热病兽)咬伤或接触病动物的排泄物、分泌物而被感染。
人和动物可患同一种病,但病理改变、临床表现和作为传染源的意义不相同。如患狂犬病的狗可出现攻击人和其他动物的行为,成为该病的传染源之一,而人患此病后临床表现为恐水症,不再成为该病的传染源。
传播途径 指病原体自传染源排出后,在传染给另一易感者之前在外界环境中所行经的途径。一种传染病的传播途径可以是单一的,也可以是多个的。传播途径可分为水平传播和垂直传播两类。
由于生物性的致病原于人体外可存活的时间不一,存在人体内的位置、活动方式都有不同,都影响了一个感染症如何传染的过程。为了生存和繁衍,这类病原性的微生物必须具备可传染的性质,每一种传染性的病原通常都有特定的传播方式,例如透过呼吸的路径,某些细菌或病毒可以引起宿主呼吸道表面黏膜层的型态变病毒,刺激神经反射而引起咳嗽或喷嚏等症状,藉此重回空气等待下一个宿主将其入,但也有部分微生物则是引起消病毒系统异常,像是腹泻或呕吐,并随着排出物散布在各处。透过这些方式,复制的病原随患者的活动范围可大量散播。
