| 重症联合免疫缺陷病 (severe combined - immunodeficiency disease, SCID) 由 T 细胞、 B 细胞均发育缺陷引起, 也可由原发性 T 细胞发育缺陷伴继发性 B 细胞功能障碍所致, 患者表现出严重、持续的病毒感染和机会感染, 患者若不慎接种卡介苗、麻疹和风疹等 减毒活疫苗, 可致全身弥漫性感染而死亡。若不进行移植, 患者一般 1～2 岁夭折。 (一) 腺苷脱氨酶缺陷和嘌呤核苷磷酸化酶缺陷 腺苷 脱 氨 酶 ( adenosine deaminase, ADA) 缺 陷 和 嘌 呤 核 苷 磷 酸 化 酶 ( purine nucleotide phosphorylase, PNP) 缺陷导致 2 - 脱氧腺苷、 S - 腺苷同型半胱氨酸和 dATP 或 鸟苷、脱氧鸟苷和 dGTP 在细胞中堆积, 这些堆积物对早期 T 细胞、B 细胞有毒性作用, 使其不能发育为成熟 T 细胞、B 细胞, 导致 T 细胞、B 细胞联合免疫缺陷。腺苷脱氨酶 缺陷和嘌呤核苷磷酸化酶缺陷均为常染色体隐性遗传。常染色体隐性遗传中约 50% 为 腺苷脱氨酶缺陷, 而嘌呤核苷磷酸化酶缺陷较少见。 (二) 细胞因子受体链基因突变 约有半数 T 细胞和 B 细胞均缺损的重症联合免疫缺陷病由 IL - 2, IL - 4, IL - 7,IL - 9, IL - 15 的受体共有的γ链 (γс) 突变引起, 因其中的 IL - 2 受体能刺激不成熟的 胸腺细胞发育, 故γс的突变将导致 T 细胞的分化障碍。患者 B 细胞正常, 但由于 B 细 胞不能获得 T 细胞的辅助, 故患者同时表现细胞及体液免疫缺陷。该突变所致的重症联 合免疫缺陷病为 X 性连锁遗传病。 (三) T 细胞信号转导缺陷 Jak - 3 激酶是 T 细胞发育成熟过程中的一种酶, 如果编码该酶的基因突变, 将导致 T细胞分化、发育障碍, 而 B 细胞发育正常。但同样由于 B 细胞不能获得 T 细胞的辅 助, 故患者仍表现为联合免疫缺陷。该缺陷所致的重症联合免疫缺陷病为常染色体隐性 遗传病. | |||||||
